Please find attached a prep package for the first FDD/SSWWE Subcommittee meeting to be held Monday, July 14, 2014 at City Hall from 6:30 pm to 8:30 pm. Per the June 18 FDD/SSWWE CAC meeting summary, the subcommittee will meet to complete the FDD survey investigation root cause analysis started by OHM. Subcommittee volunteers include Colin Breed, Vince Caruso, Judy Hanway, Peter Houk, George Johnston and Jim Osborn, The subcommittee's objective is to develop alternative recommendations/solutions for issues raised in the investigation in particular and in the surveys in general. The subcommittee's conclusions will be ultimately presented to the public for feedback -currently scheduled for Wednesday, Sept. 3 (location TBD).
Since the June 18 FDD/SSWWE CAC meeting, a City/Consulting group (including CDM/OHM/Famous in Your Field/Project Innovations), has met to review the investigation data and to assemble additional data for your analysis. This information is contained in the attached package in the following attachments:
#1: FDD Survey Scan - documents the City/Consulting groups review of the 77 OHM inspection sheets, with recommended solutions as appropriate. The scan begins with a list of the issues that we believe should be addressed first. Needless to say, the subcommittee may reach a different conclusion upon review of the 77 inspection sheets. The survey scan also includes preliminary findings and conclusions.
#2 OHM Inspection Sheets (77) - contains OHM summary of inspections, identifies neighborhood, installation year, etc. Actual addresses have been removed as well as any names/emails. We suggest you review these sheets prior to the first meeting and identify issues that you believe need to be addressed/discussed first as well as identifying general issues that require further discussion.
#3: FDD Program Completion Progress and Annual Completion Table - a CDM summary of the number and type of FDD installs per year.
#4: FDD Flow Chart, Post Installation Flow Chart and Survey Flow Chart - a CDM documentation (with City concurrence) of three key FDD processes.
#5: Homes Not Receiving Internal Post-Construction Interior Inspection, CDM memo authored by Mark Ten Broek on July 7, 2014 - the memo is in response to a City request to document number of homes that did not receive interior inspections and the process that CDM followed to schedule these inspections.
